Patient Forms
If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
Our office does our best to estimate what your insurance will contribute to your dental treatment. We contact your insurance company prior to your visit to prepare you with your copayment. Copayments are due on the day of your visit unless prior arrangement has been made.
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
All dental insurances are different. Since we deal with dental insurances every day, we understand first hand that dental insurance is sometimes complicated and can be confusing to our patients. We will do our best to calculate what your insurance will pay when you schedule treatment but we will not guarantee coverage. On occasion, your dental insurance may not cover treatments due to stipulations in your dental insurance contract agreement. If you have any questions, please contact our front office staff.
